Braftovi (Encorafenib) is a kinase inhibitor approved for use in combination therapies for various BRAF V600-mutant cancers:
- Melanoma: With binimetinib for unresectable or metastatic melanoma with BRAF V600E or V600K mutation
- Colorectal Cancer (CRC): With cetuximab for metastatic CRC with BRAF V600E mutation
- Non-Small Cell Lung Cancer (NSCLC): With binimetinib for metastatic NSCLC with BRAF V600E mutation
Limitations: Not recommended for wild-type BRAF tumors in melanoma, CRC, or NSCLC.
Recommended Dosage:
- Melanoma/NSCLC: 450 mg (six 75 mg capsules) once daily with binimetinib
- Colorectal Cancer: 300 mg (four 75 mg capsules) once daily with cetuximab
Take Braftovi with or without food. Avoid taking a missed dose within 12 hours of the next dose. Do not take an extra dose if vomiting occurs—resume with the next scheduled dose.
Secondary Skin Cancers: Regular skin exams recommended
Tumor Promotion in Wild-Type BRAF Tumors: Avoid use; confirm mutation status before treatment
Cardiomyopathy: Monitor cardiac function before and during treatment
Hepatotoxicity: Monitor liver function tests regularly
Serious Bleeding Events: Includes GI and intracranial hemorrhages
Ocular Toxicities: Includes uveitis and other inflammatory conditions; regular eye checkups advised
Reproductive Toxicity: Can cause fetal harm. Use contraception during and for 2 weeks after treatment. Avoid breastfeeding during and for 2 weeks after therapy
