Cosela is a cyclin-dependent kinase 4/6 (CDK4/6) inhibitor approved to reduce the incidence of chemotherapy-induced myelosuppression in adult patients receiving:
- Platinum/etoposide-containing regimen, or
- Topotecan-containing regimen
for extensive-stage small cell lung cancer (ES-SCLC).
Recommended Dosage:
- 240 mg/m² IV infusion over 30 minutes
- Complete infusion within 4 hours before chemotherapy
- Administer on each day chemotherapy is given
- Do not exceed 28 hours between doses on consecutive days
If a dose is missed, skip chemotherapy for that day. Resume Cosela + chemotherapy the next scheduled day.
Injection Site Reactions: May cause phlebitis or thrombophlebitis. Monitor for redness, pain, or swelling at the infusion site.
Hypersensitivity Reactions: Facial edema, urticaria, or anaphylaxis may occur. Discontinue immediately if severe reactions present.
Interstitial Lung Disease (ILD)/Pneumonitis: Can be fatal. Monitor for symptoms like cough, dyspnea, and hypoxia. Discontinue in moderate-to-severe cases.
Fetal Harm Risk: Can harm an unborn baby. Use contraception during treatment and for 3 weeks after the final dose.
Breastfeeding Restriction: Do not breastfeed during treatment and for 3 weeks post-final dose.
