Pepaxto is an alkylating anticancer agent used in combination with dexamethasone for the treatment of adult patients with relapsed or refractory multiple myeloma (RRMM). It is indicated for patients who have previously received at least four lines of therapy and whose disease is refractory to one immunomodulatory agent, one proteasome inhibitor, and one CD38-directed monoclonal antibody.
Recommended Dosage:
- 40 mg intravenously over 30 minutes on Day 1 of each 28-day cycle
- Dexamethasone 40 mg orally or intravenously on Days 1, 8, 15, and 22
- For patients aged 75 and above, the dexamethasone dose should be reduced to 20 mg
Thrombocytopenia: Monitor platelet counts during treatment, especially during the first two months. Do not initiate treatment if platelet count is below 50 x 10⁹/L.
Neutropenia: Regularly assess neutrophil counts. Do not administer if ANC < 1 x 10⁹/L.
Anemia: Monitor red blood cell levels. Dose modifications or delays may be required based on severity.
Secondary Malignancies: Long-term use may increase the risk of acute leukemia or myelodysplastic syndromes. Monitor patients accordingly.
Embryo-Fetal Toxicity: May cause fetal harm. Women of reproductive potential should use effective contraception during and for 6 months post-treatment.
Breastfeeding: Avoid breastfeeding during and for 7 days following the last dose due to potential serious adverse reactions in the infant.
