Folotyn (Pralatrexate) is a folate analog metabolic inhibitor used for the treatment of relapsed or refractory Peripheral T-cell Lymphoma (PTCL).
It was approved by the U.S. FDA on September 24, 2009 under accelerated approval based on overall response rate. Continued approval may be contingent upon further clinical trial outcomes.
Mechanism of Action:
Pralatrexate inhibits dihydrofolate reductase (DHFR), disrupting DNA synthesis and leading to cell death in rapidly proliferating cancer cells.
Indicated For:
- Adult patients with relapsed or refractory PTCL, particularly after failure of other systemic therapies.
Recommended Administration:
- Folotyn is administered intravenously, typically once weekly for 6 weeks in a 7-week cycle.
- Supplementation with folic acid and vitamin B12 is required to reduce toxicity.
- Dose modifications may be necessary based on blood counts and adverse effects.
Mucositis: Severe and sometimes fatal mucositis has occurred. Start folic acid and vitamin B12 supplementation prior to treatment.
Myelosuppression: May cause neutropenia, thrombocytopenia, and anemia. Monitor CBC regularly.
Hepatotoxicity: Elevated liver enzymes and bilirubin have been reported. Monitor liver function tests periodically.
Renal Impairment: Patients with renal dysfunction may experience increased toxicity. Dose adjustments are required.
Dermatologic Toxicity: May cause exfoliative dermatitis, rash, or other skin reactions. Discontinue in case of severe skin events.
Embryo-Fetal Toxicity: Pralatrexate may cause fetal harm. Effective contraception is required during and after treatment.
