Treosulfan is an alkylating agent used in oncology and hematology, primarily as part of conditioning regimens before allogeneic hematopoietic stem cell transplantation (HSCT).
Indications:
- In combination with fludarabine, it is indicated for conditioning treatment before HSCT in adults and in pediatric patients older than 1 month with malignant and non-malignant diseases.
- Sometimes used in cases of advanced ovarian cancer, myelodysplastic syndromes (MDS), or acute myeloid leukemia (AML) when deemed appropriate by a specialist.

Mechanism of Action:
Treosulfan acts by inducing DNA damage, leading to apoptosis in rapidly dividing cells, making it effective for cancer pre-conditioning.
Administration:
Treosulfan is reconstituted and administered intravenously under the supervision of a qualified healthcare professional. Dosage and cycle frequency are determined by body surface area (BSA), age, and underlying condition.
Myelosuppression: Treosulfan causes dose-dependent suppression of bone marrow activity. Regular monitoring of complete blood count (CBC) is essential.
Fertility and Reproductive Risk: Treosulfan can affect fertility and may cause fetal harm. Contraception must be used during treatment and for some time after.
Hepatic and Renal Function: Monitor liver enzymes and renal parameters during treatment. Use with caution in patients with hepatic/renal impairment.
Infection Risk: Patients are at higher risk for infections due to immunosuppression. Prophylactic antimicrobials and regular monitoring are recommended.
Neurological Effects: Rare cases of neurotoxicity and encephalopathy have been reported—any signs of confusion or seizures warrant immediate evaluation.
Allergic Reactions: Hypersensitivity reactions can occur. Immediate discontinuation and supportive treatment are required in case of reaction.
