Andexanet Alfa is a recombinant modified human Factor Xa (FXa) protein used to reverse life-threatening or uncontrolled bleeding in adult patients receiving FXa inhibitor anticoagulants such as apixaban or rivaroxaban.
Recommended Dosage:
- Low Dose Regimen:
- IV bolus: 400 mg at 30 mg/min
- Continuous IV infusion: 4 mg/min for up to 120 minutes
- IV bolus: 400 mg at 30 mg/min
- High Dose Regimen:
- IV bolus: 800 mg at 30 mg/min
- Continuous IV infusion: 8 mg/min for up to 120 minutes
- IV bolus: 800 mg at 30 mg/min
Administration Note: Use an in-line 0.2 or 0.22-micron filter. Do not mix with other medications.
Thromboembolic Risk: Monitor patients for arterial/venous thrombosis and resume anticoagulation therapy as soon as clinically appropriate.
Heparin Interaction: Do not use Andexxa/Ondexxya prior to heparin therapy; may reduce heparin effectiveness.
Infusion Reactions: Observe patients for mild/moderate reactions. Interrupt or slow infusion rate as needed.
Pregnancy & Breastfeeding: Not recommended during pregnancy. Discontinue breastfeeding during treatment.
Contraception Required for women of reproductive potential during therapy.
