Ninlaro is a proteasome inhibitor used as part of a combination therapy with lenalidomide and dexamethasone for treating adults diagnosed with multiple myeloma who have undergone at least one prior treatment.
Recommended Dosage:
- Ninlaro: 4 mg orally once a week on Days 1, 8, and 15 of a 28-day cycle
- Lenalidomide: 25 mg daily on Days 1 to 21 of each cycle
- Dexamethasone: 40 mg on Days 1, 8, 15, and 22
Ninlaro should be taken either 1 hour before or 2 hours after food. The capsule must be swallowed whole with water, without crushing or chewing. If vomiting occurs after taking the capsule, the dose should not be repeated.
Platelet Monitoring: Thrombocytopenia is a common side effect. Platelet counts should be checked monthly, with more frequent testing during the initial three treatment cycles.
Thrombotic Microangiopathy: Serious conditions like HUS/TTP may develop. Discontinue if diagnosed.
Liver Toxicity: Monitor liver function closely. Dose modification may be necessary for hepatic side effects.
Pregnancy Risk: Ninlaro can cause fetal harm. Avoid use during pregnancy. Male patients should use effective contraception during treatment and for 90 days afterward.
