Cablivi is a von Willebrand factor (vWF)-targeting antibody fragment prescribed for adult patients diagnosed with acquired thrombotic thrombocytopenic purpura (aTTP). It is administered alongside immunosuppressive therapy and plasma exchange to help prevent the formation of microvascular blood clots.

Recommended Dosage:

  • Day 1: 11 mg intravenous bolus injection at least 15 minutes before plasma exchange, followed by 11 mg subcutaneous injection after plasma exchange

  • During plasma exchange: 11 mg subcutaneous injection once daily after each session

  • Post-plasma exchange: Continue with 11 mg subcutaneous injection once daily for 30 days following the last plasma exchange

  • Extended therapy: May be extended by at least 28 additional days in patients with persistent signs of aTTP


Treatment should be discontinued if a patient experiences more than two recurrences of aTTP during therapy.

  • Bleeding Risk: Cablivi may increase the risk of severe bleeding. Suspend treatment if clinically significant bleeding occurs. Avoid using the drug within 7 days before elective surgical or dental procedures.

  • Hypersensitivity: Not recommended for individuals with known hypersensitivity to caplacizumab or its excipients. Reactions may include urticaria.

  • Pregnancy: The medicine may raise the risk of bleeding complications in both mother and fetus. Use during pregnancy should be guided by a healthcare provider.
