Livmarli (Maralixibat)

Livmarli (Maralixibat)

  • Medicine Name: Livmarli
  • Generic Name: Maralixibat
  • Dosage Form & Strength: Oral Solution – 9.5 mg/mL
  • Manufactured By: Mirum Pharmaceuticals, Inc.

Livmarli is an FDA-approved IBAT inhibitor used for the treatment of cholestatic pruritus (itching) in individuals diagnosed with Alagille Syndrome (ALGS), aged 12 months and older.

This rare liver disorder often leads to persistent itching due to bile acid buildup. Livmarli significantly reduces this symptom, improving quality of life for pediatric patients.

Recommended Dosage:

  • Start with 190 mcg/kg orally once daily, 30 minutes before the first meal

  • After 1 week, increase to 380 mcg/kg once daily, as tolerated

  • For patients over 70 kg, the maximum daily dose is 3 mL (28.5 mg)

  • If taken with bile acid-binding resins, maintain a 4-hour gap between medications

  • If a dose is missed and remembered within 12 hours, take it immediately; otherwise, skip the missed dose
  • Liver Monitoring: Conduct liver function tests before and during treatment. Interrupt or discontinue Livmarli if liver enzyme abnormalities persist or worsen.

  • Gastrointestinal Side Effects: Vomiting, diarrhea, and abdominal pain are common. Monitor hydration and consider dose adjustments if needed.

  • Vitamin Deficiency: Livmarli may interfere with the absorption of fat-soluble vitamins (A, D, E, K). Monitor levels regularly and supplement as required.

  • Not studied in cirrhotic ALGS patients – use caution.

To legally import Livmarli to India, the following documents are required:

  • A valid prescription from a licensed medical practitioner

  • Diagnostic reports confirming ALGS

  • Government-issued ID proof of the patient
