Nextstellis is the first and only FDA-approved oral contraceptive that combines drospirenone (a progestin) with estetrol (a plant-derived estrogen) to prevent pregnancy in women of reproductive age. Estetrol (E4) is a unique natural estrogen with tissue-selective activity, supporting uterine and bone health while minimizing liver and breast impact.

Important Note: Efficacy may be reduced in individuals with BMI ≥ 30, and it is not recommended for use in women with BMI ≥ 35 due to limited data.

Dosage:

  • One active pink tablet daily for 24 days, followed by

  • One inactive white tablet daily for 4 days, for a total 28-day cycle

  • Take at the same time each day, with or without food


Missed Dose Protocol:
  If vomiting or diarrhea occurs within 3–4 hours of ingestion, or if two or more active tablets are missed, follow backup contraceptive recommendations and resume the regular schedule.

  • Thrombotic Risk: Avoid in women with history or high risk of venous or arterial thrombotic disorders.

  • Migraine with Aura: Contraindicated in those with migraine with aura due to increased risk of stroke.

  • Hypertension: Avoid in uncontrolled hypertension; monitor BP in all users and discontinue if necessary.

  • Hormone-Sensitive Cancers: Not advised in women with current or past hormonally sensitive malignancies.

  • Liver Concerns: Avoid in patients with acute hepatitis or decompensated cirrhosis. Monitor liver enzymes if elevated.

  • Irregular Bleeding: Common during the first 3–4 months of use. Persistent or post-stabilization bleeding requires evaluation.
