LONSURF is an oral chemotherapy medicine combining a nucleoside metabolic inhibitor (Trifluridine) and a thymidine phosphorylase inhibitor (Tipiracil). It is FDA-approved for use in:

  • Metastatic Colorectal Cancer (mCRC):
    For adult patients previously treated with fluoropyrimidine-, oxaliplatin-, irinotecan-based chemotherapy, anti-VEGF therapy, and, if RAS wild-type, anti-EGFR therapy. May be used alone or with bevacizumab.

  • Metastatic Gastric or Gastroesophageal Junction Adenocarcinoma:
    For patients treated with two or more chemotherapy lines, including fluoropyrimidine, platinum-based agents, and either a taxane or irinotecan. Applicable for those previously treated with HER2/neu-targeted therapy.


Recommended Dosage:
35 mg/m² (based on Trifluridine component) taken orally twice daily with food on Days 1–5 and Days 8–12 of each 28-day cycle. Maximum dose: 80 mg per administration.

  • Bone Marrow Suppression: May cause neutropenia, anemia, or thrombocytopenia. Monitor blood counts before and during treatment.

  • Pregnancy & Breastfeeding: LONSURF may cause fetal harm. Avoid during pregnancy and breastfeeding. Use effective contraception during treatment and for 6 months afterward.

  • Hepatic Impairment: Use with caution. Limited data available. Monitor liver function regularly.

  • Renal Impairment: Requires close monitoring and potential dose adjustments due to increased risk of toxicity.
