Rolvedon (eflapegrastim-xnst) is a long-acting granulocyte colony-stimulating factor (G-CSF) used to lower the risk of infections caused by febrile neutropenia in adults receiving myelosuppressive chemotherapy for non-myeloid cancers.

Limitations of Use: Rolvedon is not approved for mobilization of peripheral blood progenitor cells for hematopoietic stem cell transplantation.

Recommended Dosage:

  • 13.2 mg via subcutaneous injection once per chemotherapy cycle

  • Administer approximately 24 hours after cytotoxic chemotherapy

  • Avoid use within 14 days before or 24 hours after chemotherapy
  • Splenic Rupture: May occur after treatment. Monitor patients reporting left upper abdominal or shoulder pain.

  • ARDS (Acute Respiratory Distress Syndrome): Assess respiratory symptoms promptly. Discontinue if ARDS develops.

  • Severe Allergic Reactions: Includes anaphylaxis. Do not administer to patients with a known allergy to eflapegrastim, pegfilgrastim, or filgrastim. Permanently discontinue in case of hypersensitivity.

  • Sickle Cell Crisis: Patients with sickle cell disorders should be closely monitored. Discontinue if a crisis occurs.

  • Leukocytosis: White blood cell count may exceed 100 x 10^9/L. Monitor CBC and discontinue if levels are excessive.

  • Thrombocytopenia: Platelet counts should be monitored throughout therapy.

  • Secondary Malignancies: Cases of AML and MDS have been reported in patients undergoing chemo/radiotherapy for lung or breast cancer. Regular monitoring is advised.
