Ocaliva (Obeticholic Acid) is an FXR (farnesoid X receptor) agonist prescribed for adults with primary biliary cholangitis (PBC) who either:
- Have not responded adequately to ursodeoxycholic acid (UDCA) after at least 12 months, or
- Are unable to tolerate UDCA.
It is used alone or in combination with UDCA in patients without cirrhosis or with compensated cirrhosis and no evidence of portal hypertension.
Recommended Dosage:
- Initial Dose: 5 mg once daily (with or without food) for the first 3 months.
- Titration: If tolerated and needed, increase to a maximum of 10 mg once daily.
For patients using bile acid binding resins, take Ocaliva at least 4 hours before or after the resin.
Liver Function Monitoring: Use cautiously in patients with cirrhosis. Ocaliva may worsen liver function and lead to hepatic failure. Discontinue if signs of decompensation occur.
Severe Itching (Pruritus): May affect quality of life. Consult your physician if symptoms become intolerable.
Cholesterol Levels: Ocaliva may reduce HDL-C. Monitor lipid profiles periodically.
Pregnancy Risk: May cause harm to an unborn baby. Effective contraception should be used during treatment and for 7 days after.
Breastfeeding: The effect on breast milk is unknown. Avoid breastfeeding during and for 7 days after the last dose.
