CAPRELSA (Vandetanib) is a kinase inhibitor indicated for the treatment of symptomatic or progressive medullary thyroid cancer (MTC) in patients with unresectable locally advanced or metastatic disease.

Recommended Dosage:

  • 300 mg taken orally once daily

  • Can be taken with or without food

  • Tablets must not be crushed

  • If needed, tablets may be dispersed in 2 ounces of water and consumed immediately or administered via nasogastric/gastrostomy tubes.

Do not take a missed dose within 12 hours of the next scheduled dose.

  • Baseline Monitoring: ECG, potassium, magnesium, calcium, and TSH should be assessed at treatment start, at 2–4 weeks, 8–12 weeks, and every 3 months

  • QT Prolongation: Avoid use with drugs that prolong QT interval

  • Severe Skin Reactions: Includes Stevens-Johnson syndrome. Therapy should be discontinued in severe cases

  • Photosensitivity: Can persist up to 4 months post-treatment

  • Interstitial Lung Disease (ILD): May be fatal. Discontinue if ILD is confirmed

  • Cerebrovascular Events: Discontinue if severe ischemic stroke occurs

  • Heart Failure: Monitor closely; discontinue if non-reversible

  • Severe Hemorrhage: Contraindicated in patients with recent hemoptysis

  • Thyroid Function: Monitor TSH levels and adjust thyroid replacement therapy if required

  • Hypertension: May require dose interruption or discontinuation

  • Pregnancy Warning: Vandetanib may cause fetal harm. Avoid during pregnancy
