Farydak (panobinostat) is a histone deacetylase (HDAC) inhibitor indicated for the treatment of multiple myeloma in patients who have received at least two prior treatment regimens, including bortezomib and an immunomodulatory agent (such as lenalidomide or thalidomide).
It is used in combination with bortezomib and dexamethasone and has been shown to extend progression-free survival in relapsed/refractory multiple myeloma patients.

Recommended Dosage:

  • Starting dose: 20 mg orally, once every other day (Days 1, 3, 5, 8, 10, 12) of a 21-day cycle, for up to 8 cycles

  • May be continued for an additional 8 cycles if well tolerated and clinically beneficial

  • Total duration: up to 16 cycles (48 weeks)

  • Administered with bortezomib (1.3 mg/m² IV) and dexamethasone (20 mg orally)


Missed doses may be taken within 12 hours; do not repeat doses after vomiting.

  • Severe Diarrhea: Monitor hydration and electrolytes (potassium, magnesium, phosphate). Begin anti-diarrheal agents at first sign.

  • Cardiac Risks: Monitor ECG periodically due to risk of ischemic events and arrhythmias. Correct electrolyte imbalances.

  • Bone Marrow Suppression: Includes thrombocytopenia, neutropenia, and anemia. Perform CBC weekly and monitor closely in elderly patients.

  • Infection Risk: May increase risk of bacterial, viral, and fungal infections. Monitor patients and initiate prompt anti-infective treatment when needed.

  • Liver Dysfunction: Regularly monitor liver enzymes and bilirubin during therapy.

  • Pregnancy Warning: Can cause fetal harm. Contraindicated in pregnant women. Women of reproductive potential should use effective contraception.
