Oncaspar is a long-acting asparagine-specific enzyme used as part of combination chemotherapy for the treatment of:

  • First-line Acute Lymphoblastic Leukemia (ALL)

  • ALL in patients with hypersensitivity to native E. coli-derived asparaginase


Mechanism of Action:
  Oncaspar works by depleting asparagine—a vital amino acid needed by leukemia cells—thereby starving cancer cells and inhibiting their growth.

Recommended Dosage:

  • Administer Oncaspar 3,750 IU/5 mL via intramuscular or intravenous injection, under supervision of a qualified oncologist.

  • Dosage is calculated based on body surface area and treatment protocol.

  • After administration, patients should be observed for 60 minutes in a facility equipped to handle anaphylaxis and infusion-related reactions.
  • Anaphylaxis Risk: Patients should be monitored closely after infusion. Resuscitation equipment and emergency medications (e.g., epinephrine, corticosteroids, antihistamines) must be readily available.

  • Thrombosis: Discontinue Oncaspar in case of serious thrombotic events, such as central nervous system (CNS) thrombosis or sagittal sinus thrombosis.

  • Pancreatitis: If patients present with abdominal pain, assess for signs of pancreatitis and discontinue treatment if confirmed.

  • Glucose Intolerance: Monitor blood sugar levels; some cases may be irreversible.

  • Coagulation Abnormalities: Monitor coagulation parameters at baseline and throughout treatment.
