PADCEV (Enfortumab Vedotin-ejfv) is a Nectin-4–targeted antibody-drug conjugate used in the treatment of locally advanced or metastatic urothelial cancer (mUC).
Approved indications include:
- Monotherapy: For adult patients with locally advanced or metastatic urothelial cancer who have been previously treated with both platinum-containing chemotherapy and a PD-1/PD-L1 inhibitor.
- Combination Therapy with Pembrolizumab: For adult patients with mUC who are ineligible for cisplatin-based chemotherapy.
Recommended Dosage:
- As a single agent: 1.25 mg/kg (max 125 mg) on Days 1, 8, and 15 of a 28-day cycle
- In combination with pembrolizumab: 1.25 mg/kg on Days 1 and 8 of a 21-day cycle
Treatment continues until disease progression or unacceptable toxicity.
Severe Cutaneous Reactions: Rare but serious cases of Stevens-Johnson Syndrome (SJS) and Toxic Epidermal Necrolysis (TEN) have been reported. Monitor for skin changes.
Hyperglycemia & Diabetic Ketoacidosis: May occur even in non-diabetic patients. Monitor glucose levels closely.
Pneumonitis / Interstitial Lung Disease: Watch for symptoms like cough, breathlessness, or infiltrates on imaging.
Peripheral Neuropathy: Monitor for new or worsening neuropathy. Adjust or discontinue dosage as necessary.
Ocular Disorders & Extravasation Risk: Eye monitoring is advised. Ensure proper IV administration to avoid tissue injury.
