Iclusig (Ponatinib) is a tyrosine kinase inhibitor approved for the treatment of adults with:
- Chronic Phase (CP) Chronic Myeloid Leukemia (CML) resistant or intolerant to at least two prior TKIs
- Accelerated Phase (AP) or Blast Phase (BP) CML or Philadelphia chromosome-positive (Ph+) Acute Lymphoblastic Leukemia (ALL) where no other TKIs are indicated
- T315I-positive CML or Ph+ ALL

How it works:
Ponatinib targets the BCR-ABL1 tyrosine kinase, including the T315I mutation, a common cause of resistance to previous CML treatments.
Dosage:
- CP-CML: Start at 45 mg once daily with or without food. Reduce to 15 mg upon achieving molecular response (BCR-ABL1 ≤1%).
- AP-CML, BP-CML, Ph+ ALL: Recommended dose is 45 mg once daily. Treatment continues until disease progression or unacceptable toxicity.
Dose adjustments should be made based on patient response or occurrence of adverse events.
Arterial Occlusive Events (AOEs): Life-threatening AOEs have been reported. Monitor closely and interrupt or discontinue if necessary.
Venous Thromboembolic Events (VTEs): Includes cases of pulmonary embolism and deep vein thrombosis. Stop therapy immediately in severe cases.
Heart Failure: Cases include decreased ejection fraction and left ventricular dysfunction. Discontinue in severe or worsening cases.
Severe Hemorrhage: May include gastrointestinal or intracranial bleeding. Monitor platelet counts; discontinue in life-threatening cases.
Hepatotoxicity: Regular liver function monitoring is advised. Interrupt or discontinue in cases of acute liver damage.
Myelosuppression: Monitor CBC routinely and adjust dosage for neutropenia, anemia, or thrombocytopenia.
Fetal Harm: Ponatinib can harm an unborn baby. Effective contraception must be used during treatment.
Breastfeeding: Not recommended while on Ponatinib due to potential harm to the infant.
