Zynlonta is a CD19-directed antibody-drug conjugate indicated for the treatment of adult patients with:

  • Relapsed or refractory large B-cell lymphoma (LBCL) after two or more lines of systemic therapy
  • Includes:
  • Diffuse large B-cell lymphoma (DLBCL) not otherwise specified

     

  • DLBCL arising from low-grade lymphoma

     

  • High-grade B-cell lymphoma

Recommended Dosage:

  • Administered via intravenous infusion over 30 minutes on Day 1 of each 3-week cycle

  • Cycle 1–2: 0.15 mg/kg every 3 weeks

  • Subsequent cycles: 0.075 mg/kg every 3 weeks

  • Premedication: Start dexamethasone 4 mg (oral or IV) twice daily for 3 days, beginning the day prior to Zynlonta treatment
  • Effusions and Edema: Monitor for signs of pleural effusion or generalized edema. Initiate appropriate medical treatment.

  • Myelosuppression: Common adverse effects include thrombocytopenia, neutropenia, and anemia. Regularly monitor complete blood counts (CBCs).

  • Infections: Serious or fatal infections may occur. Evaluate and manage new or worsening signs of infection promptly.

  • Severe Cutaneous Reactions: Monitor for rash, photosensitivity, or worsening dermatologic conditions.

  • Embryo-Fetal Toxicity: Zynlonta contains a genotoxic payload (SG3199) and may cause fetal harm. Contraceptive measures are recommended during treatment and for a period after final dose.
